Ellen was extremely concerned about this patient and continued to follow up with the SW although the SW determined the patient had a safe place to go upon discharge.
Ellen was caring for a patient who had housing and financial difficulties. Ellen was extremely concerned about this patient and continued to follow up with the SW although the SW determined the patient had a safe place to go upon discharge. The patient’s mother came to pick up the patient and newborn for discharge to home but got into a fight on the unit and again when Ellen accompanied the patient to her mother’s vehicle where her other children were. Ellen did not feel the patient was safe or had a stable home to go to with the rising heat. Ellen offered the patient to come back into the hospital unit until housing could be secured. The patient declined and drove off with the mother. Ellen informed SW and called the patient to follow up on her status. Upon answering the phone, the patient was arguing with her mother and told Ellen her mom was dropping her off at a motel, but she didn’t think she had enough money to stay. Ellen was also concerned about the 14-year-old child as there was a comment made that she didn’t want to stay with her mother and the grandmother would not take her in. The concern was that the child would be a high-risk runaway. Again due to the extreme heat and potential for this family to not have a safe home with air and running water, Ellen offered further assistance to this family and ensured our SW followed up immediately to secure housing and get this family the help and counseling needed.
