It is with great pride and enthusiasm that I nominate Eisha Tyson, RN, BSN, Charge Nurse at the Oakwood VA Clinic, for the Daisy Leader Award in recognition of her exceptional contributions to Veteran care, clinical excellence, and team leadership. Ms. Tyson consistently exemplifies the highest standards of nursing practice and leadership within our team.
Despite managing a demanding workload that includes overseeing day-to-day clinic operations, mentoring peers, precepting new staff, and serving as a resource for both clinical and administrative needs, she continues to deliver outstanding outcomes in population health management. Most notably, she leads the charge in improving and sustaining top-tier HEDIS performance metrics, including maintaining the highest scores among all seven PACT teams for critical measures such as HbA1c control, BP <140/90, renal testing, BP (18-85 DM), statins and vaccinations.
Her proactive approach to population health, meticulous use of CPRS tools, and compassionate Veteran engagement have played a pivotal role in driving these results. Ms. Tyson demonstrates:
• Exceptional clinical skills and critical thinking in chronic disease management
• A deep commitment to Veteran-centered care and health equity • Proficiency in managing complex workloads without compromising care quality
• Effective interdisciplinary collaboration with providers, pharmacists, and case managers
• Consistent leadership during team huddles, performance reviews, and high-risk patient interventions
She models VA’s I CARE values daily, and her work directly supports our facility’s goals in achieving High Reliability Organization (HRO) standards. Her ability to lead by example, uplift her team, and maintain performance excellence makes her truly deserving of this recognition. She is an exceptional VA RN who continues to elevate the care we provide to our nation’s Veterans.
