My mom had several trips to the ED, MD office, and 4East. There were several great nurses in all of the areas! But the nurse who stood out was Diana. 

We had been struggling with multiple continuity of care issues. Communication between different physician teams. Ruling out differential diagnoses... And communication with the patient and family about the plan. 

This nurse listened!  And then followed through with each of the items that needed addressing, despite resistance in some areas.  It was the first time since the original office MD visit that we felt heard.  And then to have things acted upon in a timely manner, with follow-up communication, even when things didn't flow well, was so healing. It helped my mom not worry and maybe my family as well. But I know it helped her finally relax. In addition, this nurse asked questions in a way that Mom could understand at 91 and probed a little differently to get the answers she needed to provide the necessary care. 

Diana was patient, a great listener, a collaborative team member, and true patient advocate! We were so appreciative of her care of our mom and Grammie!!
