I am honored to nominate Collin for the DAISY Award in recognition of his exceptional care and compassion toward my grandma. During her time under Collin’s care, my family and I witnessed a level of dedication and kindness that truly went above and beyond. Collin listened to us as a family and helped advocate for my grandma’s needs.

There was one particular thing that stood out to me. My grandma, who was often anxious and confused until Collin's care, had no diagnosis. I had mentioned that she needed a SLUMS eval, as I believed my grandma had advanced dementia. It was not until Collin, who really heard me and advocated, saying he would talk to the doctor to see if that could be ordered while she was in house. Because of Collin, my grandma was able to get the proper testing, and we were able to get a diagnosis that helped put all the puzzle pieces together. If it were not for Collin, my grandma would have been discharged and diagnosed and would not have gotten the help that she needed.

Beyond the emotional support, Collin demonstrated extraordinary clinical expertise. He kept my grandpa well-informed about my grandmother’s condition and treatment plan, ensuring we were always comfortable with the decisions being made. His attention to detail and proactive care gave our family peace of mind, knowing that my grandmother was in excellent hands.

If my grandma were in her lucid mind, she would have loved Collin, and I wish he could have met her at that. I want to thank you for caring for my grandma and listening to our family.
