Because of Colleen's determination and sense of responsibility, this patient received life-saving intervention.
Colleen was following up on a complicated post-op patient who had cancelled a follow-up appointment. Knowing that the patient was discharged home on fluconazole due to a fungal infection, she was concerned about his INR management. During her chart review, she found some concerning lab values and a note from an OSH ED. Due to these findings, Colleen immediately ran to the ART clinic to speak with the team directly and arrange getting the patient admitted to MUSC.
On arrival at MUSC, the patient's INR was 20. The patient was transferred to the CTICU for cardiogenic shock management, as well as metabolic derangements, including acidosis and renal failure. Colleen's proactive vigilance, clinical attention, and unwavering advocacy for patient safety directly impacted this patient's outcome. Colleen recognized a life-threatening situation that had been missed, took swift initiative to escalate care, and worked collaboratively with multiple teams to ensure the patient received the urgent treatment needed.
