Each team member came and made sure that I was okay.
After a recent hospitalization, I missed a scheduled appointment with this team that I needed for a chronic illness.
This team checked my medical record to see why I had missed the appointment and saw the note that I had been admitted to a non-VA hospital. They tracked my community care until I was discharged.
The following Monday morning, they called me to see how I was doing and to schedule me as soon as possible for my missed appointment.
The entire team was committed to making sure that I didn't have any lapse in my care. The team worked hard to make sure that I could be scheduled on a day that I was already going to be seen by my primary care provider.
When I arrived, the team walked me down the hallway. Each team member came and made sure that I was okay.
The attention I received proved to me that I am not just another patient to them, but someone they know they are helping.
The team gives each of us Veterans individual care and watches out for us during our appointments.
If our assigned nurse is not around, we know another is right there to assist us if we need it.
