This telemetry team exemplifies the spirit of the DAISY Award through their unwavering commitment to compassionate, high-quality patient care and their dedication to improving outcomes for every patient they serve. Their teamwork, innovation, and patient-centered approach make them a true asset to our hospital and community.
The nurses and care partners on this unit consistently go above and beyond for their patients. Every day, they demonstrate kindness, empathy, and professionalism. Whether it’s hearing nurses patiently teaching nursing students, or care partners sharing laughter with patients who need a moment of joy, this unit radiates compassion and teamwork. It is truly inspiring to witness the care and camaraderie that fill this floor.
Recently, the team cared for an elderly woman admitted following surgery. During her stay, she confided in the nurses that she did not feel safe or well cared for in her current living situation. She shared her wish to return to her own home in another state, where she had medical equipment and support waiting. The nursing staff listened attentively, validated her concerns, and immediately began advocating on her behalf. They collaborated with physicians and care coordination to explore safe discharge options, and with help from a neighbor who confirmed access to her home, they arranged home health services and ambulance transportation to make her return possible. When the patient was discharged, she was overcome with emotion and gratitude, expressing how thankful she was to finally be going home.
This unit’s excellence extends beyond individual acts of compassion—it’s built into their culture. They maintain one of the most active and thriving Shared Leadership Councils in the hospital, driving continuous improvement and accountability.
A few examples of their outstanding teamwork include:
• Improving Communication: Care partners developed a handoff solution within EPIC to ensure that every team member received consistent, critical patient information at shift change, including diet, fall risk, Braden score, and blood sugar orders.
• Accountability and Excellence: This team was the only unit in the hospital to achieve 100% completion of annual competencies before the deadline—a testament to how they hold each other accountable and strive for excellence.
• Technology and Quality Improvement: Working with the EPIC team, they utilized the SlicerDicer tool to identify barriers and improve real-time patient scanning compliance and pain documentation.
• Patient Experience: Their efforts are reflected in outcomes—they recently earned the best patient experience scores hospital-wide for the first quarter of the fiscal year (July–September).
• Clinical Projects: They significantly reduced hypoglycemic events through a focused quality improvement project and enhanced heart failure management by improving intake and output documentation, optimizing diuretic therapy, and strengthening patient education to reduce readmissions.
Every member of this team embodies professionalism, accountability, and compassion. Their collaborative spirit and commitment to excellence inspire those around them and positively impact every patient they touch. I am incredibly proud to witness the extraordinary care and teamwork that define this unit. They are truly deserving of the Team DAISY Award recognition.
