How to Add a Unit
- Use the Active scope dropdown at the top of your dashboard to select the facility where you want to add a unit.
- Click Units in the left navigation menu.
- Review the Current Units list to make sure the unit has not already been added.
- Click + Add Unit in the upper-right corner.
- Enter the unit’s name in the Unit name field. Each unit name must be unique within that hospital.
- Click Add Unit to save. The new unit is created with an Active status.
Adding multiple units? You can also use CSV Import on the Units page to upload multiple units from a CSV file.
How to Add a Nurse
- Use the Active scope dropdown to select the correct facility.
- Click Nurses in the left navigation menu.
- Check whether the nurse is already listed. Enter their first name, last name, or email address in Search directory, then click Apply filters.
- If the nurse is not already listed, click + Add Nurse in the upper-right corner.
- Enter the nurse’s First name and Last name.
- Enter their Email address if available. This field is optional.
- Select their unit from the Unit dropdown, or leave it Unassigned. Only active units can be selected.
- Click Create nurse to save the nurse to your facility’s directory.
Why add nurses? Maintaining your nurse directory and assigning nominations to the correct nurse helps your DAISY Program track nomination counts accurately.
Important: Adding a nurse to the directory does not create a user account or give them access to your dashboard.
How to Assign a Nomination
- Use the Active scope dropdown to select the correct facility.
- Or click the tray icon at the top of the dashboard and open the nomination you want to assign.
- Review the Submitted answers, including the nominee’s name and nomination story, to identify the correct nurse.
- In the Assigned Nurse section, open the Assign a nurse dropdown.
- Select the correct nurse from your facility’s directory.
- Click Assign to save the assignment. Confirm that the nurse’s name appears under Assigned Nurse.
Nurse not listed? Add them through the Nurses section first, then return to the nomination to complete the assignment.
Why assign nominations? Linking each nomination to the correct nurse helps your DAISY Program track how many nominations each nurse has received. Assignment does not change the original submitted answers.
Exporting Your Nominations
- Use the Active scope dropdown to select the facility whose nominations you want to export.
- Click Form activity in the left navigation menu.
- Locate the nomination form you want to work with. Click View submissions if you want to review its nominations first.

- Click Export CSV at the bottom of the panel to download the file.
