Say Thank You to Your Nurse! Nominate Them for The DAISY Award® for Extraordinary Nurses!
Thank you for taking the time to nominate your nurse! Below, you can choose the healthcare facility where your nurse works and access the online form. Your expression of gratitude is so important and will go a long way in helping them cope with high stress, burnout, and compassion fatigue.
Since 1999, DAISY has been honoring nurses who provide above-and-beyond compassionate care to patients and families. Read our story of why DAISY was created here.
What happens after you submit a nomination:
Your nomination will go directly to the DAISY committee at the healthcare organization. Depending on how that organization has structured its DAISY program, the committee will review the nominations monthly, quarterly, or annually and select an Honoree.
Every nurse who is nominated for The DAISY Award will receive a copy of their nomination, a nominee pin, and a congratulations note.
Nurses who are selected for the Award will receive a copy of your nomination, an Honoree pin, an Award certificate, a Healer’s Touch sculpture, a banner to hang in their unit, and cinnamon rolls or a cinnamon treat to share. Learn more about the process here.

Finding Your Healthcare Facility
- Enter your state from the drop-down list below. You may also add in your city to further narrow down your search.
- Click Apply.
- Locate your facility in the results list below, click the name in green, and you will be redirected to their nomination form. Please include as much detail as possible, not only about what your nurse did that made a difference but also how your nurse’s care made you feel. If there is more than one nurse you would like to nominate, please list them on the form with their names separated by commas. DAISY also has a Team Award that honors nurse-led teams.
