The Veteran and his wife were very thankful, Taj did not stop there. She followed up with the Veteran after one month to make sure he was doing okay.
Our 72-year-old Veteran with undiagnosed Alzheimer's disease came into the office for an appointment but he could not tell us why his appointment was made. The scheduler had made the appointment as a two-week follow-up visit from the eye clinic. The Veteran told the PACT provider that he thought he was going to have cataract surgery today and he was going to meet an ophthalmologist. This added to the confusion, Taj looked through the Veteran's chart and told the Veteran he had an active consult for ophthalmology and we do not do surgeries in the outpatient office. The veteran was still confused, Taj reviewed the office notes from the eye clinic but did not see where he needed to be seen by primary care. She went the extra mile to track down the optometry resident physician who had evaluated this Veteran and clarified the need to be seen in the primary clinic. It was determined that the Veteran needed a thorough neurological examination. Taj was able to complete the Neurological evaluation and ordered a CT of his brain to rule out a stroke. She then placed a referral to a neurologist to address the frequent falls and cognitive decline. The Veteran and his wife were very thankful, Taj did not stop there. She followed up with the Veteran after one month to make sure he was doing okay. She offered him additional resources with the social worker.
