Nomination #1

A very young child was admitted to Home Care for skilled nursing services related to her new diagnosis of cancer. Mary Jo admitted the child and spent much time clarifying the IV infusions and setup that the child came home with. Mary Jo took the extra time to assure that everything was set up correctly and worked with the parents to teach them how to manage the infusions at home. Another visit was necessary on that Saturday night as the parents were not able to retain all the information in just the start of care visit. Another nurse was on-call that Saturday night when the visit needed to occur. Mary Jo contacted the other nurse and offered to go with her to the visit due to the complexity of the IV infusions and set up. Mary Jo felt it would be best for the child and her parents as it would be reassuring to have the same nurse there again to go through everything with the parents again and the other nurse who would ultimately be the child's case manager.

Mary Jo did go with the other nurse and they were in the child's home until very late into Saturday night. Mary Jo's actions demonstrated her commitment to the patient and the child's family as well as to her co-worker. She would not have had to take this action but did so because of her dedication to her work as a nurse and knowing that this would make a real difference in the lives of the parents and child as well as her coworker. Mary Jo very quietly did this without the need for recognition for her efforts. This is something that Mary Jo does every day in her work with this example demonstrating how she goes above and beyond for the sake of our patients' and coworkers.



Nomination #2

Mary Jo is an admission nurse in our Home Care Department. She is a quiet stable RN in our fast-paced department. Mary Jo helps out in so many ways and recently she worked doing 2 admissions and a visit, took call that night - was out all night with our patients and then worked the next day. Mary Jo does her job with excellence, she advocates for our patient and will diligently continue with the follow up of her admissions when she could have passed the work to someone else. She is a gentle and kind spirit and keeps working diligently everyday she is here. She is a great nurse and I am proud to call her my friend.

Mary Jo works well with all the Home Care staff. It is important to be able to tie all disciplines - whether it is O.T, P.T., Social Workers, Secretary, or Nursing. She is consistently building excellent care plans for her admission team to follow.
