Dan McMahon, Diabetes Nurse Educator, goes above and beyond to care for his patients. Last week, he was working to discharge a Type I Diabetic man who had numerous re-admissions related to his mismanagement of his diabetes. The patient was homeless and did not have a secure plan in place to store his insulin in a refrigerator. Therefore, the patient would not receive insulin and would ultimately return to St. Mary's requiring hospitalization. Dan worked with the patient, his family and care management to devise a discharge plan in which the patient would be able to refrigerate his necessary insulin in hopes of avoiding future readmissions along with better overall management of his chronic condition. Dan advocated for St. Mary's to purchase a mini refrigerator so the patient could store his insulin. This is just one example of why Dan is the quintessential DAISY Nurse.
