You will need to sign a liability from acknowledging that The DAISY Foundation has no liability in your safe experience on the medical mission. The link to this form can be found below.
Following Your Mission Trip
Within 30 days of your return from your mission trip, to receive reimbursement, you will need to complete the reimbursement form (also linked below). This form will ask you to:
- Describe the impact of your medical mission and your contribution to it.
- Upload photos for our use in publicity of your experience and future promotion of this grant program.
- Provide the receipts for your expenses that we will reimburse.
