Valerie contacted the PNH Bariatric Services Department seeking any available assistance in obtaining the needed equipment and resources to care for a severely obese patient (>500 lbs.). This patient was in ICU and intubated. Resources for her proper care absolutely were going to play a factor to the degree she recovered - or not!
Valerie participated in the Critical Care rounds and when discussions occurred about withholding tube feedings for the patient due to the fear of diarrhea and the difficulties that would then ensue to turn and clean the patient, Valerie lobbied for the proper, necessary nutrition for this patient, but she didn't stop there. Part of the staff's concerns with repositioning the patient was not having the proper bed for the patient nor the proper turning equipment. Also, some staff had already voiced issues with back pain after caring for this patient. Valerie contacted the wound care nurse and the proper bed was ordered, then together, Valerie and I met with the Wound Care Director to discuss processes for future patients. Proper turning sheets, wedges, hover mats, etc. were also discussed and obtained. Proper fitting gowns were added to the linen cart in ICU as well as house wide, so this patient and future patients would have properly fitting gowns.
Valerie kept contact with the staff and then kept me updated with the equipment status and issues. In just over a week, this patient was extubated and alert and oriented with an appropriate conversation with work being done for her rehab placement.
I truly believe Valerie made a tremendous difference in this patient's medical course and outcome with the dedication and effort she put forth to ensure all that could be done was being done!
