Nomination Letter for Patti Eason, RN, Case Management

Patti Eason is the evening case manager and covers the entire hospital. She is always helpful and checks with the floors to see if they need any help with late discharges or other problems within her scope of responsibility.

We had a patient on the 4th floor to be discharged but there were problems that needed to be resolved first. One problem was that there was no electricity in the house where he stays. He is oxygen dependent and he needed power for his oxygen concentrator. MLG&W was called during the day and they said they turned the power on. The other problem was that none of his family would return our calls so that we could verify that someone would be home to let him in when he arrived.

Patti was aware that we had been trying to get his discharge accomplished for 2 days! She took charge of this situation and got it done. She called the police and asked if they would go over to his house to be sure that someone was home to let the patient in and that they had electricity. Family members were home but just not answering their phones! She also checked with her supervisor and got permission to transport the patient by ambulance rather than another transport service. If there were a problem when they got to the house, the ambulance would bring the patient back to the hospital when other transportation would not do so. The ambulance crew would be able to be sure that the house was safe for the patient and be sure the oxygen machine was working properly before they left. She made calls and worked on this case all evening until we got the patient discharged about bedtime. She went out of her way to take care of the needs of this patient and made sure he got home safely. I nominate her for the DAISY Award!
