Meredith Carr exemplified good clinical sense and outstanding nursing work ethic. As a float nurse, Meredith happened to be in the ER. Our patient presented to the ER with shortness of breath, especially worse with exertion. Patient had a history of sarcoidosis but was limited to her skin. She was very functional and actually drove to the hospital from work. The patient was clearly dyspneic, tachypneic, and tachycardic. Initially the plan was for the patient to go home, however, Meredith took it upon herself to reevaluate the patient before discharge, she was found to be hypoxic with O2 sats around 88% at rest. We ended up admitting the patient and she was found to have a large saddle embolism. Meredith exhibited exemplary clinical acumen, which is the mark of a very good nurse. She was able to get a sense that the patient was not well and that something else was going on and needed further evaluation. This is a main feature of what I expect out of a good nurse. That she or he is able to tell when a patient is sick and needs further evaluation.
