When Melinda Carlton was working in Nurse Visits, Population Care Management staff stated that they were thankful she was working in Nurse Visits. She was the only one addressing all the member's POEs when she did the glucometer downloads. Previously, staff had stated they were too busy to do POE's for PCM. We rolled out the practice of all staff working in Nurse Visits after she demonstrated that it was possible to meet member's needs in that way. Melinda will not sit idle if no members are scheduled, she will jump into message management, stocking, or find something to stay busy. By going above and beyond Melinda demonstrates her commitment to excellence.
When Melinda first moved from Nurse Visits to a module working with one physician, I let him know he was getting a new nurse. He was hesitant. Recently he came and told me he is very pleased, that she is wonderful to work with and gives great care to our members. Melinda has displayed exemplary service as demonstrated by her first quarter ASQ score of 100% for Nurse Care and Concern with a decent sample size. She is very compassionate when working with members showing kindness and her caring heart.
Melinda is gentle with our members. When doing direct report rounding when I asked her peers who they wanted to recognize for going above and beyond I received some of the following comments:

Since day 1 Melinda always goes beyond for everything.

I can just say, 'Can you please do this Melinda?' and she just does it, she doesn't give a thousand reasons why she can't and doesn't question it with 'Why me?

Melinda just jumped into Module 2 and helps whenever she can.

Melinda is always there to help whenever I ask. If she sees me drowning, she just helps without being asked.

Melinda is a great teammate, always open to help, she is new to the module and is a great addition.

Melinda is a positive influence in the module and is committed to excellence with the attitude that all the members are our members, and we need to work as a team to help in whatever way we can. Recently she floated to a different module and I received e-mails from everyone in the module asking if she could come and work in their module! No matter what task Melinda is given she demonstrates her flexibility in always making time to get extra assigned tasks accomplished, whether it be working on message management between patients, or rolling out new practices which she masters and then teaches others. Through these actions, she demonstrates exemplary leadership skills.
