As the home health liaison, I assess patients coming into home health services, the degree of acuity some of our patients can be overwhelming sometimes. Jeremy takes the more difficult patients with as much ease as the less acute. I often call field staff to give verbal reports. When I speak with Jeremy, his response is always a positive one. The last time he stated, "we are really going to help this patient, he needs us." He has given 110% every day with his patients and same goes for his coworkers.
Jeremy is one of those people who strive to make the world around him a better place for everyone. He is a bright light to many of our patients and to all of his coworkers. His positive attitude and smile brighten everyone's day. He is always looking to improve situations of his patients the best he can. Recently Jeremy accepted some difficult patients knowing ahead of time the challenges he was going to face. Without hesitation stating, "I can make this better for the patient." His first patient was a former patient who was very unhappy with Ministry and was actually going to go with a different home care provider. But because of insurance coverage and the other provider not being covered, was thinking about not receiving home care at all. Jeremey met with this patient and his wife prior to admission to let them know that this time would be different for them. He goes above and beyond to make sure that all staff are aware of what is happening with his patient and keeping the patient's wife informed as well. Jeremy was open to any phone calls and questions they may have regarding the patient's treatment plan. So far things are going wonderfully.
The second patient is not only a patient with a lot of health issues but also family issues. Before admission the patient was in the hospital, the social worker from the hospital who was making the referral had stated that the patient's girlfriend didn't want him home as "he was no longer an asset". As this referral was being discussed, could we provide the type of care this patient was going to need? Jeremy was an advocate for this patient stating, "he needs us" and he was more than willing to take this patient.
Jeremy goes the extra mile for his patients and our community. He is always willing to pick up extra patients to make sure that everyone gets the care they need and deserve. He is a wonderful nurse, patient advocate and overall example of what we all should strive to be.
