Yunah truly went above and beyond to create a safe transfer from the MICU. The patient that was being transferred was a psych patient with a history of violence during the current admission. In order to ensure that this transfer was safe, Yunah called and requested to give an early report, which would allow us time to get organized on our floor. This allowed us to free up a cognitive recovery room, which was the appropriate placement for a patient with this history. Additionally, she had extensive documentation regarding how to talk to the patient, and how to get cares accomplished given this patient's labile emotions. She spent 15 minutes discussing how they had worked with the patient, and what was effective.
Prior to transfer, Yunah came up to the unit with the patient's food tray, to ensure that the patient had a positive experience when arriving on our unit. She suggested items to discuss with the doctors prior to admit in order to make sure that there were appropriate orders if there was difficulty with the transfer. As a result of Yunah's preparation, the patient transferred to the floor without incident and had a calm admission prior to discharge. The preparations that had been discussed allowed us to work with staff to interact with the patient in a manner that allowed them to remain calm, and have productive interactions. The advanced notice also allowed the receiving medicine team to be prepared for the transfer, and safely make a place of care. Yunah's diligence and effort showed a gold standard of patient transfers, and let me feel comfortable taking an admit and providing safe care with minimal resources over a weekend.
