Patty was the Charge Nurse in the CICU recently when a care conference was held regarding one of our patients who had a need for intense neuro rehab. The patient was here with his mother, but they are from out of state. Mother had expressed to multiple people on the care team that she needed to get back to work in order to keep the family's finances stable. The patient's cardiac issues had been resolved, and the issue was whether or not we could send the patient to TX (closer to home) for neuro rehab so that Mom could get back to work.
Patty initiated contact with the pediatric facility in TX and contacted several different people/institutions to find out the best way to go about transferring this patient once the care team was comfortable doing so. Patty had to speak with several different people to find out the necessary answers, but she continued to seek out these people in order to get all of the information needed. She was able to provide complete information to the care team regarding transferring the patient to a facility closer to home, which streamlined communication among the team and eliminated the need for others to seek out this information.
