The family of a dying patient had many instances where there was little or no communication during his hospitalization. Patient had been in ICU over 30 days, was transferred to 6th floor (family not notified) then transferred to 7W (again no notification) then to 7E as a hospice patient. Patient's planned DC to adult foster home was stopped when patient suffered an acute change in mental status; unit moved him to another room and then the bed broke. Overall, the entire scenario was horrible for this patient and his family. Mary provides the compassion to patients and families at the end of a patient's life. She kept the family informed, was extremely supportive when dealing with the appropriate frustrations. She called them in when she identified that the patient was nearing the end. They were so appreciative that after all of the missteps, they were able to be here with him as he passed away. Mary consistently is recognized by patients and families for her care, more so for her caring.
