I am nominating Jeri Revels for the DAISY award due to her extraordinary caring and compassion for our patients as demonstrated in the following scenario.

Last summer, while working as a home care nurse, Jeri was in her car getting ready to go to her next patient's home. She was driving on 18th St near the Bettendorf library which is a busy thoroughfare, when she noticed a man in his sixties on foot, wandering and weaving in the middle of the street. He appeared confused and was certainly in harm's way. Jeri pulled over, got out of her car, and approached the man. As she talked with him, Jeri found he had been to a doctor's appointment and was planning to take the bus home but had run out of money. He was attempting to walk home but was confused about how to get there. He lived by himself and did not have anyone to call for help. Using her assessment skills, Jeri noticed this man didn't seem quite right. Jeri called her nursing supervisor, explained the situation, and asked permission to drive this man home which was about a mile away.

The nursing supervisor entered the man's name into our computer system to see if he was or had been a patient of ours in the past and it was discovered he was a current patient of Genesis VNA. Jeri received permission to drive this man to his home. After they arrived Jeri assessed him and found his heart rate was significantly elevated at about 150 beats per minute. Jeri decided she would stay with him for a while as she attempted to have the man rest and relax for a time. Perhaps the previous activity was affecting his heart rate; however, that was not the case. His heart rate remained elevated. Jeri concluded something was significantly wrong. She called 911 and continued to stay with him until help arrived. The man went to the emergency department where it was discovered he was dehydrated and was treated.

I was impressed with Jeri's keen assessment skills and her compassion to stop in the middle of her work day to help another human being. She exhibited excellent teamwork and communication skills by contacting her supervisor about the situation. I feel she is very deserving of the DAISY Award.
