We had a patient admitted to our inpatient rehab unit with a diagnosis of cancer with bone metastasis. The patient had been told he only had a few months left to live. Jennifer really went above and beyond to make the transition home as easy as possible for the patient and his wife. Everyday leading up to his discharge she would speak to the patient’s wife on the phone or in person to answer questions as well as to set-up the best discharge plan possible. She made several calls to the medical supply company to set-up the proper equipment for the patient and dealt with the insurance company calling her daily regarding his care. Jennifer sat in the patient’s room with his wife while she cried and expressed her anxiety about taking her husband home. Jennifer also made follow-up phone calls to the patient and his wife after discharge. Jennifer stayed at their side through this tough time and made a grim outlook a little easier to bear for this man and his wife. She truly exemplifies what the DAISY Award is about.
