We nominate Glynda Meadows for the DAISY Award for her outstanding and compassionate care of a patient that has been in and out of our hospital many times especially in the last year. She is a 55-year-old woman that is suffering from end stage COPD. She currently has a tracheostomy and is bedbound and ventilator dependent. This patient was able to go home on a vent because of Glynda’s determination to make it happen and has only been able to maintain at home because of Glynda’s help. She lives with some family members and with her husband who works.
The Friday before Memorial Day this patient was in the hospital and ready to discharge home on IV antibiotics. The social worker had contacted the infusion company and everything was set up until we were notified that the infusion company could not get the authorization for the IV drug. Due to the holiday, the patient would have to stay in the hospital until Tuesday. Glynda immediately got on the phone to our pharmacy and found out the drug was very inexpensive and asked the hospital to provide the IV medicine, which they did. The problem was they could only send half of the drug as it was only stable for a few days. This meant that the next week the patient would need to pick up the rest of the medicine from pharmacy. The patient has a family that is very unreliable so, of course, Glynda offered to pick it up and take it to the patient’s home.
Glynda continues to case manage this patient even when she is at home. This week she received a text message from the patient that she was unable to get her narcotic pain medicine refilled which she needs desperately. Glynda worked with the Dr. and Medicaid to clear up the problem with the medication ordered and will once again be picking up the patient’s prescription for her and delivering it to her home. Glynda is always available to answer the call when this patient runs into a problem. The patient texted Glynda today telling her how much she loves her and how grateful she is for her help. “Thank you for caring. Love you, J B”.
