The entire Team on 7 Central Tele/Med/Surg coordinated care with a family and patient that has been overly challenged with a chronic, debilitating disease. The patient, only 27 years old, has experienced a lifetime of pain, surgeries and hospitalizations. Her Mom, the Care Giver, was sure to maintain control with the 7 Central Team as they all cared for her daughter. Families that are exhausted from chronic diseases recognize the importance of keeping some routine when their loved one is hospitalized. The Plan of Care can be a challenge for both the patient and the healthcare team. After months of hospitalization, the patient and her family wrote several DAISY nominations for the entire team that worked hard to ensure the best of both worlds could be accomplished to the benefit of the patient. The patient grew and developed friendships with the team.
